Some clinicians believe the answer to back lies in your muscles. Muscles are thought to be the cause of low back pain and other common pain problems.

Pain that originates from the muscles and connective tissue is often referred to as myofascial pain; similarly, Myofascial Pain Syndrome is frequently the diagnosis used to describe the problem. Further, trigger point pain is often considered to be the same as myofascial pain, so it helps to fully understand how trigger points affect muscles and connective tissue, like tendons and ligaments. Trigger points are small nodules, or knots, in the muscle that when pressed produce pain locally and often at a distant area. For example, if you are experiencing pain in your trapezius muscle (a large muscle that supports the upper arm) you may experience trigger point pain that radiates to the upper arm. Trigger point pain may even lead you to believe that the pain you are feeling originates in the upper arm, rather than the trapezius! Doctors inject trigger points, often with varying results.

When trigger point injections are given in hopes of relieving muscle pain, there are several inconsistencies:

  • Doctors choose from several different drugs, including lidocaine, steroids, botox, or sometimes nothing (known as dry needling.)
  • Needle length varies from very short (5/8 in) to long (3 ½ in.)
  • The injection site may be just the painful nodule in the muscle or the entire muscle, including the areas where the muscle attaches to bone or fascia.
  • After injections, patients generally receive some type of recommendation for exercise.

Since muscle injections can vary greatly depending on the clinician giving them, there is currently a variety of research that seems to be unable to determine if muscle injections are a valid approach to the treatment of chronic low back or neck and shoulder pain.
