  • Facet Joint Osteoarthritis – Osteoarthritis, also referred to as degenerative arthritis, can cause breakdown of cartilage between the facet joints. When the joints move, deficiency of the cartilage causes pain as well as loss of movement and stiffness. The facet joints are found in the back portion of the spinal column. The joints combine with the disc space to create a three-joint complex at each vertebral level. The facet joint is made of two opposing bony surfaces with cartilage between them and a capsule around it that produces fluid. The amalgamation of the cartilage and the fluid allows the joint to move with very little friction. However, facet joint arthritis triggers the cartilage to breakdown and the joint movement is associated with more friction. The sufferer loses mobility and as they get stiffer they have more back pain.

  • Degenerative Spondylolisthesis – The name is actually Latin for "slipped vertebral body", and it is diagnosed when one vertebra slips forward over the one beneath it. This disorder occurs as a consequence of the general aging process in which the bones, joints, and ligaments in the spine become weak and less able to hold the spine in alignment. This disease is more common in people over 50 but most sufferers are over 65, with women being afflicted more than men.

Letting loose with a satisfying string of expletives can significantly reduce physical pain, research has shown. But according to a new study, it doesn't work as well if you make a habit of cursing frequently.

In a previous study, psychologist Richard Stephens at Keele University in the U.K. and colleagues found that people who were asked to dunk their hand in ice-cold water were better able to tolerate the pain — and they left their hands in the buckets for a full 40 seconds longer — if they were allowed to swear, compared with people who were asked to utter a non-curse word.

In this new study, Stephens repeated the previous experiment, asking 71 college students to submerge their hands in freezing water for as long as they could bear it. One group was asked to repeat a swear word of their choice — one they might use if they banged their head accidentally, for instance — while their hands were in the water. The other group was asked to repeat a control word they might use to describe a table. Then, both groups repeated the task using the word they hadn't previously tried.

The researchers found that 73% of the participants kept their hands under water longer while swearing, replicating the original finding. On average, the swearers lasted 31 seconds longer in the cold hand plunge.

Interestingly, however, the more frequently participants reported swearing during the course of their daily lives, the less effective cursing was at killing their pain and the shorter their endurance time in the cold water test.

It seems that swearing may help relieve pain by activating the brain's endogenous opioids, the natural pain-relieving chemicals whose effects on the brain are similar to pain drugs like morphine and oxycodone. As with opioid drugs, repeated swearing may increase people's tolerance to their effects, and cause them to need higher "doses" of cursing to achieve the same effect. In some sense, people may become addicted to — or at least physically dependent on — cursing.

The authors note that people who frequently express their anger verbally tend to be more sensitive to both acute and chronic pain. Indeed, research has found that these high "trait anger out" individuals also have a higher threshold for triggering endogenous opioid action in the brain. In other words, unless these people vigorously express their anger verbally, they feel more pain.

Although the current study didn't look at people who have this temperament, it's possible that those who tend to swear or express their anger frequently are trying to relieve their pain and irritability. They could become more outwardly angry over time, in order to chase opioid pain relief, which they increasingly become more tolerant to. Such individuals may also be especially susceptible to opioid addiction, and if they become addicted, they may be good candidates for long-term maintenance on the drugs to relieve this pre-existing opioid system dysfunction.

The annual American College of Rheumatology Conference was held two weeks ago, and though I was not in attendance, I learned that some intriguing information was revealed.

Much news, information, and research was shared, but there were a few key elements that could mean good things for those with rheumatoid arthritis. Though the information has been around for a while, it was further elaborated upon,So let's learn more about these potential advances in RA research.

JAK Inhibitors:

If you have Rheumatoid Arthritis or a similar rheumatic condition, you have probably heard about Anti-TNFs (also called TNF-blockers) and also treatments that affect the B-cells.

A newer class of biologics that work differently than the existing ones are called JAK Inhibitors, and they are at present being further looked into. According to the ACR and MedPage Today,

"JAK has attracted increasing attention as a drug target in rheumatoid arthritis because it's central to the inflammatory response — mediating the release of tumour necrosis factor and other cytokines — and because, as a kinase enzyme, it should be amenable to blockade by an oral, small-molecule drug."
The emergence of the JAK kinase as the new hot target in rheumatoid arthritis has been further cemented
"with positive phase II data on an oral drug that may compete with the field's current leader."
Tony James from The Rheumatology Update agrees.
"The novel oral JAK inhibitor tofacitinib significantly improves the signs and symptoms of RA in patients unresponsive to other TNF inhibitors, a phase III study shows."

Rheumavax Vaccine:

Promising news included a vaccine designed to re-educate the immune system. According to a press release issued from the ACR Conference,

"A vaccine designed to re-educate the immune system has been shown to be safe and feasible in the treatment of rheumatoid arthritis
, according to research presented this week at the American College of Rheumatology Annual Scientific Meeting in Chicago.
'RA is caused by an aberrant response that can be triggered by disease-specific antigens, or proteins,'
says lead investigator Ranjeny Thomas, MBBS MD; professor of Rheumatology at The University of Queensland Diamantina Institute.
'Current treatments attempt to reduce the joint destruction associated with this disease by targeting the inflammation that is a consequence of this immune response. However, non-specific suppression of inflammation may increase infection, and drugs may be toxic to vital organs.'

Dr. Thomas' team recently completed a study looking at the feasibility and safety of using a vaccine, called Rheumavax®, to reduce the way a person's immune system reacts towards the disease antigens thought to cause RA.

'Approximately 70 to 80 percent of RA patients make antibodies towards RA-specific antigens called citrullinated peptides, which are identifiable with a blood test,'
explains Dr. Thomas.
'While antigen-specific immune tolerance is a major goal for RA prevention and treatment, early trials failed to show a response, and the strategy was abandoned when biologic therapy was spectacularly successful. We reasoned the problem might be solved if the dendritic cells that normally educate the immune system towards these antigens were modified with a drug that would promote immune tolerance.'
And, with that thought in mind, Dr. Thomas' team designed Rheumavax using drug-modified dendritic cells (generated from a person's own blood) and citrullinated peptide antigens, to suppress specific RA immune reactivity (or restore immune tolerance) – leaving broader immunity towards infections intact."

Betalains are a coloured pigment found in the Caryophyllale family of plants and in some higher order fungi. The name is derived from the Latin name for beets, Beta Vulgaris and are what gives theses roots there distinctive red colour. There are two main classes of Betalains. Never underestimate the power of setting goals. I am not referring just to a target weight. Although it is helpful to have a long-term goal to set your sights on, something so far in the future has done very little to keep me motivated and on track from day-to-day. It is even more important to set medium- and short-term goals, too. Medium-term goals are milestones along your weight-loss journey. For example, every ten pound loss, or every time you drop a jeans size. I used 10% targets for my medium-term goals. In other words, I celebrated each time I lost 10% of my total body weight. But remember, weight loss goals do not have to be tied to the scale at all. Good goals are anything that motivates you, as long as the goal is SMART (specific, measurable, attainable, realistic, and timely). Short-term goals can actually be daily or weekly goals. My daily goals include eating at least five servings of fruits and/or veggies, drinking at least eight glasses of water, and writing in my journal. My weekly goals include exercising at least 3 days per week and walking/running at least 5 miles per week. Choosing goals is a highly personal experience. My goals work for me but may not float your boat at all. So it's critical to find goals that are things you can personally achieve, and that support your longer-term goals. Once you have decided on your goals, be sure to put them front-and-center. Post them where you will see them often and be reminded of them every day. You might also want to come up with some way to track your progress toward your goals. For instance, every time I drop a pound, I place a little footprint sticker on a poster hung on the wall by my bed. Watching the number of footprints grow has been really motivating for me and helps keep me focused on my goals.

One great stretch is to lay flat on your back and pull your knee into your chest, one leg at a time. Hold this stretch for at least 30 seconds at a time. Afterwards, bring both legs together and pull into your chest and hold. This stretch will help relieve the tension in your lower back. Another great stretch that will relieve pain in your quadriceps is to start by laying flat on your stomach. Then, bring both of your legs up to your buttocks and grab on to your feet with your hands. Try holding for 10-20 seconds or until you start to feel pain. Child's pose is also a great stretch to help loosen and relieve your lower back pain.

It is important to keep you back flat when doing any of these stretches.

Core Strength - Crunches are a great way to help build up your core to help support your lower back. You need to be careful when doing crunches. If you feel any pain at all, stop.

Trying these light exercises and stretches will help reduce your pain and get you on the right track to recovery. Remember to start out slow when exercising with sciatica and always consult your doctor.

Cracking Your Knuckles does not cause Arthritis, its a myth. You would think that popping your knuckles would eventually cause so much repetitive trauma that it would lead to arthritis, but there have been studies done and there is no correlation between cracking your knuckles and getting arthritis.

So what causes the "popping" sound?

Your knuckles are joints covered by a capsule (the joint capsule). Within this capsule there is fluid, which acts as a lubricant and also contains nutrients for the adjacent bone surfaces. A variety of gases are continuously dissolved in this fluid. When one cracks a knuckle, the stretching of the capsule lowers the pressure inside the joint and creates a vacuum, which is filled by the gas previously dissolved in the fluid. This creates a "bubble" which then bursts producing the "popping" or "cracking" sound. It takes a while until these gases are re-dissolved in the fluid, which explains why knuckles cannot be "re-cracked" immediately.

Side effects to cracking knuckles?

There is no evidence that cracking knuckles causes arthritis. However, there are studies associating knuckle cracking with injury of the ligaments surrounding the joint or dislocation of the tendons.

What causes arthritis?

There are different kinds of arthritis with the major categories being two: The inflammatory type such as rheumatoid arthritis and the degenerative type best known as osteoarthritis (wear and tear). Tendons- Tendons transmits the force from the muscle to the bone. Tendons have much more tensile strength and much more resistant to injuries involving over stretching . More common pathologies of tendons are tendinitis and tenosynovitis.
  1. Tendinitis: - Chronic use of the tendons results in chronic stress which results in the breakdown of the collagen.Swelling of the tendon due to chronic overuse results in the decrease in the collagen.Treatment involves use of anti inflammatory drugs and rest . Resting the effected tendon will help in regeneration of the collagen .Rehabilitation, in many cases, continues to focus on anti-inflammatory treatment strategies, rather than collagen rebuilding. In some cases, the use of anti-inflammatory medication, such as corticosteroid injections or oral anti-inflammatory medications, can be detrimental for healing collagen degeneration. Overuse tendon disorders can take a long time to heal due to the slow rebuilding of collagen. Collagen rebuilding is a slow process.

Tenosynovitis - Tendons in the extremities , usually in the arms and the legs will have protective covering called as synovial sheath . chronic overuse and the friction in the synovial sheath results in the inflammation of the synovial sheath .   3. Avulsion results from an acute tendon injury resulting from high tensile loads, in which a tendon is forcibly torn away from its attachment site on the bone. In some other cases these fibers remains intact and the tendon pulls away from its bony attachment site.Avulsion injuries occur in regions where a large muscle attaches at a relatively small site on the bone, such as the hamstring attachment. These injuries requires surgical management.

Heart disease is the number one cause of death in the United States. There is a growing body of research demonstrating that cholesterol levels are no longer the best indicator of a person's risk for a heart attack or stroke. In fact, half of the people who suffer a heart attack have completely normal cholesterol levels. Recent studies have shown that lowering cholesterol alone had no benefit on preventing a person's first heart attack or stroke. Medical research is showing that far better indicators for risk of heart attack or stroke are oxidative stress, size of lipid particles, and inflammation status.

Cholesterol

Eighty percent of the cholesterol in your blood was actually produced by your own body, the other twenty percent comes from the diet. As more cholesterol is consumed in the diet, the body makes less. Studies have actually shown very little correlation between the amount of cholesterol consumed in the diet and blood cholesterol levels because of this regulatory system. That is not to say that eating an unhealthy diet full of greasy burgers and french fries will not lead to heart disease, quite the contrary. But the cause is not about the amount of cholesterol so much as how these foods effect blood sugar regulation, fat deposition, inflammation, lipid particle size, and oxidization of fats.

Oxidative Stress

When oxygen is processed by the body, reactive oxygen species (free radicals) are formed. Under healthy conditions, the body neutralizes the majority of these metabolites with circulating antioxidants. When there is not a sufficient quantity of antioxidants available to limit oxidative damage, reactive oxygen species can cause damage to local tissues. In the cardiovascular system this is especially important because when vessels are damaged plaques can form leading to atherosclerosis and heart disease.

Methods of reducing oxidative stress include eating a colourful whole foods diet, reducing inflammatory substances like trans-fats, cigarettes, alcohol and sugar and, when necessary, taking antioxidant supplements.

Size of Lipid Particles

Low Density Lipoproteins (LDL) are often referred to as the "bad cholesterol." Under oxidative or inflammatory conditions, LDL particles can embed into vessel walls and form plaques that lead to heart disease. Gathering evidence is demonstrating that assessing the size of LDL particles is a far better predictor for heart disease than the shear amount of LDL in the blood. Smaller denser LDL particles have been strongly correlated with heart disease risk where larger "fluffier" LDL particles are not. Two people with the same LDL levels can have much different risks of developing atherosclerosis depending if their LDL particles are small and dense or large and fluffy.

Inflammation

Inflammation is the body's attempt to repair. Think about what the healing process looks like when you cut yourself. The skin gets red around the cut and slowly over time lays down new tissue until eventually the wound is closed and healed. The same process happens when internal organs experience injury from extraneous sources. A poor diet, chronic stress, smoking, inactivity, and poorly controlled blood sugar are just a few sources of chronic inflammation and repair deficit which greatly increase the chance of developing atherosclerosis and heart disease.

What is arthritis? If you decipher the word, "arthr-" means joint and "-itis" means inflammation. So, arthritis is just inflammation of the joints. Is inflammation good or bad? Yes and no. It pumps the chemicals and agents related to inflammation out of the joint so that healing can occur.
