Insulin resistance is a classic pre-cursor to diabetes, as well as a frequent companion to Syndrome X.

Is there a link between chronic sinusitis and heart disease? Dr. Stefan Kiechl, a professor of neurology at Innsbruck University Clinic in Austria, belives so.

Kiechl and his colleagues studied people who suffered from chronic sinusitis (sinus inflammation and infections), they found that an inordinate amount of them also ended up with clogged arteries and microvascular heart disease. This was despite each patient's otherwise positive living habits (good dietary habits, good weight maintenance, moderate stress levels, and few family backgrounds of heart disease).

Their conclusion was that when the body's immune system senses the inflammation, it sends out immune cells. However, the bodies of people who suffer from heart ailments often over react and send out more immune cells than are needed to fight the inflammation. This causes the sinuses to become blocked. When the inflammation turns into an infection, the body sends more white blood cells.

Now there are so many white cells roaming around that they don't know what to do with themselves, so some of them go after the fat cells in the body's arteries or microvessels. When the white cells try to pull these fat cells out through the cell wall, they find they're too big and so they get caught. Over time, these trapped immune and fat cells build up in the artery walls and become calcified, blocking the blood flow or even breaking off into dangerous clots.

Dr. Kiechl said,

"Among subjects with chronic infections, the risk seems to be higher in those with a prominent inflammatory response. We are not sure yet whether it is because of the risk associated with the toxic effect of the inflammation itself or the body's immune reaction to it".

Their results did show, though, that of the 826 volunteers, 268 suffered chronic infections. At the end of the five year study, the plaque levels of the patients' carotid arteries were compared to their levels at the start of the study. The study found that 332 (or 41%) had developed plaques where none previously existed or had an increase of about 28% in their plaque levels. This included all 268 of the patients who suffered from chronic infections, as well as 64 who did not. However, the patients with the highest increases or largest new blockages were the ones who had suffered from chronic bronchitis or sinusitis, although the type of sinusitis made no difference.

Dr. Kiechl said,

"It is far too early to recommend that people take antibiotics long term to suppress inflammation".
He pointed out that although several studies, including his, have linked infections with heart disease, none has shown that people who took antibiotics long term had a lower risk of heart disease. Meanwhile, if you find yourself suffering from an inflammation or infection, using an anti inflammatory is highly recommended, especially if you already have a heart condition.

Heel pain is most often caused by plantar fasciitis which is an inflammation of the band of tissue (the plantar fascia) that extends from the heel to the toes. In this condition, the fascia first becomes irritated and then inflamed, resulting in heel pain. The condition that is sometimes also called heel spur syndrome when a spur is present. Heel pain may also be due to other causes, such as a stress fracture, tendinitis, arthritis, nerve irritation, or, rarely, a cyst.

Causes
The most common cause of heel pain relates to faulty structure of the foot. For example, people who have problems with their arches, either overly flat feet or high-arched feet, are more prone to developing plantar fasciitis.

Wearing non-supportive footwear on hard, flat surfaces puts abnormal strain on the plantar fascia and can also lead to plantar fasciitis. This is particularly evident when one's job requires long hours on the feet. Obesity may also contribute to plantar fasciitis.

Non-Surgical Treatment

  • Stretching exercises. Exercises that stretch out the calf muscles help ease pain and assist with recovery.
  • Avoid going barefoot. When you walk without shoes, you put undue strain and stress on your plantar fascia.
  • Ice. Putting an ice pack on your heel for 20 minutes several times a day helps reduce inflammation. Place a thin towel between the ice and your heel; do not apply ice directly to the skin.
  • Limit activities. Cut down on extended physical activities to give your heel a rest.
  • Shoe modifications. Wearing supportive shoes that have good arch support and a slightly raised heel reduces stress on the plantar fascia.
  • Medications. Oral nonsteroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen, may be recommended to reduce pain and inflammation.

Although it's widely accepted that inflammation is a critical underlying factor in a range of diseases, including the progression of cancer, little is known about its role when normal cells become tumour cells. Now, scientists from the Florida campus of the world renowned Scripps Research Institute have shed new light on exactly how the activation of a pair of inflammatory signaling pathways leads to the transformation of normal breast cells to cancer cells.

The study, led by Jun-Li Luo, an assistant professor at Scripps Florida, was published online by the journal Molecular Cell on February 23, 2012.

The scientists' discovery points to the activation of a self-sustaining signaling circuit that inhibits a specific RNA, a well-known tumour suppressor that helps limit the spread of cancer (metastasis). Therapies that disable this circuit and halt this miRNA repression could have the potential to treat cancer.

In the new study, scientists identified the specific pathways that transform breast epithelial cells into active cancer cells.

The researchers found immune/inflammatory cells ignite the transient activation of MEK/ERK and IKK/NF-kB pathways; the MEK/ERK pathway then directs a consistent activation of a signaling circuit in transformed cells. This consistent signaling circuit maintains the malignant state of the tumour cells.

Luo compares this process to starting a car—a car battery starts the engine much like the transient signal activation turns on the consistent signal circuit. Once the engine is started, it no longer needs the battery.

The scientists go on to show that the initial activation of these pathways also activates IL6, a cytokine involved in a number of inflammatory and autoimmune diseases, including cancer. IL6 acts as a tumour initiator, sparking the self-sustaining circuit in normal breast cells necessary for the initiation and maintenance of their transformed malignant state.

In establishing that self-sustaining signal circuit, IL6 represses the action of microRNA-200c, which is responsible for holding down inflammation and cell transformation. Since enhanced microRNA-200c expression impairs the growth of existing cancer cells and increases their sensitivity to anti-tumour drugs, compounds that disable microRNA-200c repression have the potential to act as a broad-spectrum therapeutic.

Interestingly, the new findings dovetail with the "multiple-hits theory"of tumour formation, which posits that once normal cells in the human body accumulate enough pre-cancerous mutations, they are at high-risk for transformation into tumour cells. While the newly described initial pathway activation is momentary and not enough to cause any lasting changes in cell behaviour, it may be just enough to tip the cell's transformation to cancer, especially if it comes on top of an accumulation of other cellular changes.

Here is a bit of news that I did not know, Chocolate is good for your inflammation problems

Now my readers should all know by now that some inflammation is good – it's needed for wounds to heal. However, a whole bunch of bad diseases can result if your body's inflammation goes haywire. New research looked at chocolate itself, and found that small amounts of chocolate before exercising can reduce fatigue and allow you to go further. The mice in the study who were fed a purified form of the flavonol epicatechin were able to go about 50% further than the mice forced to exercise without it. Even cooler, biopsies of their leg muscles showed the chocolate-group had formed new capillaries and mitochondria as a result! The researchers suggest that a small 5 gram serving of dark chocolate, about half the size of a typical square, will help enhance your workout. This will only work with dark, since the processing for milk chocolate destroys the epicatechin.

As a former adventure racer I used to include dark chocolate in my ration packs, perhaps that's why I became a chocoholick.

Of all the claims chocolate can make for your health, the impact it can have in reducing your risk of cardiovascular disease is the most impressive and the most thoroughly studied. A recently published study reviewed several other studies and trials, including over 100,000 subjects, looking at many factors to see if chocolate had a protective effect on the heart. They found that across the board, no matter if the chocolate came from the oft-recommended dark chocolate, or if it came from cakes, cookies, drinks or even nutritional supplements, the groups that ate the highest amounts of chocolate had a 37% reduced risk of cardiovascular disease and a 29% reduced risk of stroke.

In treating cancer symptoms, we treat from the front; once symptoms have developed symptoms are attenuated, where possible, with medication. As our understanding of inflammation in symptom development increases, inflammation may provide a target in treating cancer symptoms at their genesis.

Such an approach would be of interest and potentially therapeutic value. Studies fully exploiting the pro-inflammatory response as a target in the treatment of cancer symptoms are eagerly awaited.

Many arthritis sufferers use what is known as complementary and alternative practices to manage their symptoms. Patients use alternatives for many reasons. It may be because conventional medical treatment isn't giving them the relief they need, or they may be concerned about the side effects associated with drug therapy, else they may feel that alternative therapies are safer because they are "natural." Whatever the reason, many patients are finding additional relief by incorporating these five practices into their arthritis management regimen:

  1. Regular exercise
    The National Institute of Arthritis and Musculoskeletal and Skin Diseases (NIAMS) recommends that arthritis sufferers use these types of exercise:
    • 1. Range-of-motion exercises such as dancing to help maintain normal joint movement, relieve stiffness, and increase flexibility.
    • Strengthening exercises such as weight training to help keep or increase muscle strength.
    • Aerobic or endurance exercises such as bicycle riding to improve cardiovascular fitness, help control weight, and improve overall function. Weight control is important because extra weight puts extra pressure on many joints.
  2. A healthy diet
    Since there are over 100 known types of arthritis, there is no single diet that will help everyone. However, the Arthritis Foundation has established the following guidelines to assist patients in maintaining a diet that will help manage their symptoms:
    • 1. Eat a variety of foods, and avoid those that can interact with your medications.
    • Use fat and cholesterol in moderation.
    • Eat plenty of vegetables, fruits and whole-grain products.
    • Use sugar and salt in moderation.
    • Drink alcohol in moderation.
    • Take in the daily requirements of vitamins and minerals, including calcium.
  3. Dietary supplements
    The American College of Rheumatology has noted that there is some evidence that ginger and willow bark extract may relieve pain.

    These remedies, however, may contain chemicals that are similar to those found in naproxen (Aleve) and ibuprofen (Advil), which can cause inflammation of the stomach and intestines. They also may interfere with blood clotting and cause fluid retention, which may be problematic for people with high blood pressure or cardiovascular disease.

    • Chondroitin sulfate and glucosamine supplements are sold separately or in combination, and are recommended for pain relief when used in conjunction with other pain medication. Although their effectiveness is unproven, the college says they
      "appear to be relatively safe, and may be worth considering for people who have severe pain despite conventional treatments."
    • Fish oils that contain omega-3 fatty acids have been reported to relieve pain and joint tenderness. It may take several months before a patient sees results. Side effects include gastrointestinal distress and breath odour.
  4. Manage stress
    Stress is a normal part of life; but for arthritis sufferers, there are additional stress factors like increased dependence on family members and changes in appearance due to joint deformities or the side effects of drugs.

    The Arthritis Foundation says that controlling stress is imperative for arthritis patients because

    "when you feel stressed, your body's muscles become tense. This muscle tension can increase your pain and fatigue and may limit your abilities, which can make you feel helpless."
    Stress can lead to depression, which can touch off a cycle of stress, pain, fatigue, and limited/lost abilities. Managing stress can break that cycle.
  5. Hot and cold applications
    The Mayo Clinic makes the following recommendations:
    "Heat will help ease your pain, relax tense, painful muscles and increase the regional flow of blood. One of the easiest and most effective ways to apply heat is to take a hot shower or bath for 15 minutes. Other options include using a hot pack, an electric heat pad set on its lowest setting or a radiant heat lamp with a 250-watt reflector heat bulb to warm specific muscles and joints. If your skin has poor sensation or if you have poor circulation, don't use heat treatment.

    Cold may dull the sensation of pain. Cold also has a numbing effect and decreases muscle spasms. Don't use cold treatments if you have poor circulation or numbness. Techniques may include using cold packs, soaking the affected joints in cold water and ice massage."
