When you have a chronic debilitating illness, you are not alone but have joined millions of others who all share one thing in common: - The need to learn to live with disease. Below are some suggestion to help you cope with the situation.

  • Accept that you are ill. This is the first thing you have to do. Stop here and accept your illness, and then accept that you have to learn to live with your condition. This is the first step to getting your psychology right. Everything comes easier after you come to acceptance.
  • Educate yourself about your illness. Read at least 3 books about your illness. Then go to PubMed.com and search for at least 10 articles on your illness. Knowledge is power.
  • Assemble a team clinicians. It matters who you allow to treat you. You need to find compassionate clinicians who are understanding of your illness.
  • Learn coping mechanisms for your illness. Effectively coping with your illness, your shortcomings, and your depression is a must. Keep in mind that positive actions can become habits; and positive habits are what you need to establish.
  • Take positive actions to control your life: If you have difficulty keeping commitments, create an annual schedule that highlights all of your appointments, events, and commitments, and continue to look forward to upcoming events like holidays and birthdays, and don't dread the unpleasant events.
  • Get your nutrition under control: Eat a healthy diet. Avoid bad carbs, saturated fats, cholesterol foods, fried foods, sugary foods. Be proportioned so that you are allowing your body to function normally. Fiber adds bulk to food, leaving you feeling fuller, longer.
  • Exercise: Stretch your muscles every day. Develop some kind of beneficial fitness program and stick to it, at least 3 days a week that involves endurance training, strength training, and core body strengthening with coordination.
  • Maintain good sleep architecture: Adhere to a strict sleep schedule. Go to sleep and wake at the same time every day to keep yourself on a cycle. (positive actions)
  • Do bot become isolated: It is important to maintain your social calendar. Living a structured life with commitments will keep you moving. Use whatever aids you need to in order to stay in the game (cane, cushion to sit on, orthotics in your shoes, etc.). Remain stimulated. Don't quit the hobbies that bring you pleasure.
  • Manage your wellness. You have to manage your wellness because no one else will do it for you. Medicine will only take you so far; the rest is up to you.
  • Stop making excuses: Don't let your illness control you. You are not a victim of your illness; it can only effect you to the degree you allow it to.
  • Find simple pleasures and cherish the good times: Some days, all you may have are simple pleasures. It's these types of pleasures that can carry you through the tough times. Find something to hope for.
This list is universal for most chronic illnesses. Doing all of these things doesn't cure your illness, but if you don't do them, you will feel worse, I guarantee it. It's tough, no one disputes that, but it's the right thing to do to keep you as healthy and as symptom free as possible.

Most low back pain can be treated without surgery. Treatment involves using analgesics, reducing inflammation, restoring proper function and strength to the back, and preventing recurrence of the injury. Most patients with back pain recover without residual functional loss. Patients should contact a doctor if there is not a noticeable reduction in pain and inflammation after 72 hours of self-care.

Other studies suggest that bed rest alone may make back pain worse and can lead to secondary complications such as depression, decreased muscle tone, and blood clots in the legs. Patients should resume activities as soon as possible. At night or during rest, patients should lie on one side, with a pillow between the knees (some doctors suggest resting on the back and putting a pillow beneath the knees).

Exercise may be the most effective way to speed recovery from low back pain and help strengthen back and abdominal muscles. Maintaining and building muscle strength is particularly important for persons with skeletal irregularities. Doctors and physical therapists can provide a list of gentle exercises that help keep muscles moving and speed the recovery process. A routine of back-healthy activities may include stretching exercises, swimming, walking, and movement therapy to improve coordination and develop proper posture and muscle balance. Yoga is another way to gently stretch muscles and ease pain. Any mild discomfort felt at the start of these exercises should disappear as muscles become stronger. But if pain is more than mild and lasts more than 15 minutes during exercise, patients should stop exercising and contact a doctor.

Medications are often used to treat acute and chronic low back pain. Effective pain relief may involve a combination of prescription drugs and over-the-counter remedies. Chronic inflammation is one of the causes of premature aging. It is also the reason behind the common issues we see in the aging population such as arthritis, cataracts, heart disease, obesity, autoimmune diseases, dementia and others.

Inflammation per se, is not bad. It is a process that the body uses to repair itself after experiencing damage or trauma from any cause. It is when this process gets out of control that people experience disease states. Recent studies have shown that certain cancers can be triggered by inflammatory conditions. For instance, cervical cancer can be caused by HPV. Liver cancer can be caused by hepatitis B and C. Mesothelioma can be caused by asbestos. MALT lymphoma can be caused by H. pylori.

Incorporating a plant-based diet (anti-inflammatory) goes a long way in improving inflammatory conditions. The Mediterranean, Indian and Asian cuisine are great examples of anti-inflammatory diets. Other modalities that may help inflammatory conditions include acupuncture, Ayurvedic remedies, cold-laser treatments and others.

What is Pain, normally, a pain message begins with some form of stimulation in the nerve endings. The message is transferred from the point of pain up to the spine, then to the brain where it is interpreted and a "withdrawal" strategy is formed. Other pain begins in the nerves themselves or even in the brain. In any case, pain is a serious and persistent problem. Let's discuss a few common types of pain and even some possible solutions for chronic pain.

Soft tissue pain
Muscle pain is different from nerve pain and bone pain. It usually arises because of a direct injury. Trauma to the soft tissues elicits a basic response: inflammation. The pathway of inflammation begins with the release (locally) of chemicals designed to heal the injury. These chemicals attract circulation to nourish distressed tissues and blood clotting factors in case of bleeding. They also generate a fever in the injured area to fight off infections ("bad" bacteria hate fevers!) and extra lymphatic fluid to dilute toxins and poisons and to remove the dead bacteria.

These same chemicals facilitate a pain signal that continues long after the injury has passed. The pain signal causes us to examine the injury and take steps to protect the area. These steps are explained in the Latin description of inflammation:

  • Rubor - redness associated with increased blood flow
  • Tumor - swelling associated with increased lymph flow
  • Calor - heat associated with the local fever produced near the injury
  • Dolor - the pain that alerts us to the damage
  • Functio laesa - the loss of function of the injured area, often involving muscle spasms

Unfortunately, these five steps tend to feed on themselves. It is completely possible and very common for the cycle of pain and inflammation to spawn another cycle of pain and inflammation that may continue for years! Resolving soft tissue pain involves:

  • Removing the cause of the pain
  • Breaking the cycle of inflammation
  • Promoting complete healing.
Nerve pain
As mentioned, the nerves communicate distress from injured soft tissues to the brain. However, many illnesses and certain injuries affect the nerves in such a way that they send pain signals when no injury exists. This is very much like the screech you hear when a microphone gets too close to an amplified speaker. (I hate that noise!) The pain signals feed on themselves in a way completely disproportionate to the injury - if an injury even exists in the first place!

Resolving nerve pain involves:

  • Creating healthy nerves
  • Reducing the feedback within the nerves themselves
  • Restoring function to the affected tissues
With nerve pain, certain types of exercises and massage may prove quite helpful. This is because stretching during exercise and massage break up adhesions and scars that may be trapping or even compressing a nerve, causing it to send out false distress signals. Bone pain The sensation of bone pain is very interesting, a little confusing and perplexing. You see, bones don't have many nerves in them. Yet, the pain is very real and often overwhelming. Bone pain often comes from the joints and is radiated or projected into the bones. Bone pain can also come from the soft exterior of the bone or the bone's moist interior.

Resolving bone pain involves:

  • Creating healthier joints
  • Hardening the jelly-like exterior of the bones
  • Reducing the drying effect of inflammation within the bone.
Brain pain
Your brain is essentially one big nerve. Yet it does not feel pain; rather, the brain interprets pain. Centers for pain perception within our brain interpret signals as pain. Such pain syndromes as Sympathetic Reflex Dystrophy and even Fibromyalgia may originate deep within the brain itself, often at the site of an old traumatic brain injury, and are interpreted as being somewhere else. Unless you resolve the injury and inflammation in the brain, these pain syndromes may never go away.

Conclusion
Pain signals and pain perception is designed to get our attention, would you agree? When we feel pain we automatically assume something is wrong. And something is. The different types of pain can tell us not only what is wrong but also, potentially, how to solve the problem.

Inflammation is a current medical buzzword, if you look it up on Google you will find over 56 million results.

Cereals Are Inflammatory
Breakfast cereals contribute immensely to obesity, inflammation and chronic disease. Oatmeal for cardiovascular health is a total fraud. The fiber might be useful, but the high starch causes cardiovascular disease. Grains/cereal are the foundation of the chronic disease pyramid.

Inflammation Not Serum Lipids Cause CVD: Statin Are Unnecessary
Inflammation is the source of chronic degenerative diseases. Serum lipids are only secondary factors. Statins lower serum lipids, but do not impact cardiovascular health unless they also lower inflammation. Lowering inflammation lowers serum lipids and decreases cardiovascular disease. Statins appear to be a very expensive way of treating cardiovascular disease dependent on their side effect on inflammation. Modest dietary and lifestyle changes are much more effective, cheap and safe than statins.

Reproduction Requires Controlled Inflammation
Aspirin blocks many of the steps in triggering inflammation and thus, aspirin administration can be used to reveal a role of inflammation in many unexpected places. Aspirin is effective in blocking some forms of infertility, inhibiting miscarriages and ameliorating postpartum depression. So inflammation is a critical part of reproduction. But, also notice that depression is a symptom of chronic inflammation.

Cancer Requires Inflammation
High dose (IV) aspirin has been successfully used to treat cancer. Inflammation is required for cancer growth, because both use the same transcription factor, NFkB. The aberrant signaling of cancer cells would normally lead to programmed cell death, apoptosis, but inflammation blocks apoptosis. Aspirin can in turn block NFkB and in the absence of inflammation, cancer cells die by apoptosis.

Inflammation is Self-Limiting
Aspirin also transforms the COX/lipoxidase system to produce anti-inflammatory prostaglandins/eicosinoids. Inflammation normally progresses into anti-inflammation. Blocking this progression leads to chronic inflammation and a shift from local to systemic inflammation with the rise of inflammatory interleukins in the blood stream.

Immune Response Requires Inflammation
The signal molecules (IL-1, IL-6, TNF) and transcription factor, NFkB, associated with inflammation were all initially identified in the development of lymphocytes. Hence, IL stands for interleukin, a hormone that triggers leukocyte (literally white blood cells or cells associated with the lymphatic immune system, i.e. lymphocytes) development. The nuclear factor, i.e. transcription factor, involved in expression of the large chain, kappa, of immunoglobulins in B cells, was called NFkB.

Genes Expressed by NFkB Cause Symptoms of Inflammation
About five dozen genes are under control of NFkB. Among these are COX-2, the enzyme that converts omega-6 arachidonic acid to inflammatory prostaglandins; iNOS, the enzyme that produces nitric oxide that dilates blood vessels to produce hot, red skin; and the inflammatory interleukins, IL-1, IL-6 and TNF, associated with autoimmune disease, fatigue and cachexia (wasting).

Auto immunity and Allergy Start with Inflammation
Medical treatments focus on symptom abatement and ignore cause. What causes obesity, allergy or autoimmune disease? The answer appears to be chronic systemic inflammation plus exposure to unusual proteins. The unusual proteins are immunogenic, i.e. interact with the immune system to produce antibodies or reactive T-cell receptors, and are subsequently recognized as autoantigens or allergens, that are the targets for immune attack. Inspection of these autoantigens and allergens shows that they all have one thing in common, they bind to heparin via a strong heparin-binding protein domain that is typically a triplet of adjacent basic amino acids.

Anti-Inflammatory Diet and Lifestyle Protects
Dietary and lifestyle adjustments that minimize inflammation, e.g. low starch, no HFCS, low vegetable oil (except olive) and supplements of vitamins D & C, fish oil (omega-3) and glucosamine, reduce the risk of allergies/asthma, degenerative diseases and cancers. Simple, high level supplements with fish oil reduce numerous mental disorders, e.g. depression, ADHD; infertility, pre-eclampsia and postpartum depression; allergies, asthma; arthritis, atherosclerosis; burn recovery, septicemia and head injury.

Your retirement years are supposed to be the best time in your life. You work your entire life away and save money to enjoy the final years in retirement. But, many people can't enjoy their time to the fullest because of arthritis or some other type of joint-related pain.

Dr. Harlan Krumholz of the Yale University School of Medicine and his team reviewed 30 different studies that were conducted by Merck prior to Vioxx's withdrawal in 2004. Many of them were made available after the drug was banned by the FDA. Each trial was examined, one at a time. Krumholz found a strong, undeniable link between patients taking Vioxx and increased heart attacks and strokes dating as far back as the year 2000.

This study went through a rigorous peer review process before the findings were deemed "acceptable." Then, Merck responded by issuing a statement saying, "The article published in The Archives of Internal Medicine… used unreliable methods and reached incorrect conclusions."

The Wall Street Journal published an investigative article back in 2004 reporting that it had seen internal Merck emails sent between company executives. The emails expressed concern over Vioxx and the increased risk of heart attack. They go back as far as the late 1990s.

Even aspirin and Ibuprofen have drawbacks when taken for long a long period of time, so you are better advised to seek natural pain and inflammation relief.

What is Inflammation?

Typically when we think of inflammation, we think of a body part becoming swollen from injury, like the swelling of a sprained ankle. Did you know that often taking NSAIDs such as Advil after fracturing a bone will delay the healing process. It turns out the same mechanisms your body uses to heal soft tissue injuries also apply to the regeneration of new bone.

The process of bone fracture healing can be divided into four phases: immediate, early, regenerative, and remodeling. The immediate phase alerts your body to the injury. The early phase, characterized by inflammation, creates the pain and swelling associated with the fracture and is critical to successful fracture healing. The prostaglandins produced by the body in the early phase are essential in sustaining the healing process until the bone is completely healed. The regenerative phase occurs when the fracture is bridged with new bone, and the remodeling phase is when the newly generated bone is replaced with mature bone to restore strength and integrity.

When NSAIDs are taken after a break or a fracture bone healing is inhibited because the activity of bone-building cells, called osteoblasts, is not supported by the presence of prostaglandins. Without prostaglandins, osteoblasts can still make new bone, "but not as robustly,'' said Dr. Thomas Einhorn, chairman of orthopedic surgery at the Boston University School of Medicine. The result, a bone that isn't quite as good as new.

As it turns out the old adage is right — no pain, no gain. When we use NSAIDs to block prostaglandins in an attempt to keep ourselves from feeling the pain of an injury, we are in fact stopping the healing mechanisms of the body. Inflammation is a necessary process that increases blood flow to deliver nutrients and recruit the necessary cells to clean-up and rebuild to the area of the body that has been injured. Pain is our body's way of telling us it is time to let that area of the body rest. Any medication that inhibits the inflammatory process in turn inhibits the body's ability to heal itself.

  1. How much weight are you carrying? Your knees bear the brunt of your body weight, so it's crucial that you maintain a healthy body mass index (BMI). So, if your BMI is 25 or more, you may be compromising the health of your knees. In fact, obesity is one of the biggest risk factors for developing osteoarthritis because it speeds the breakdown of cartilage. Dropping extra weight — particularly body fat — may be the single most important thing you can do to reduce the risk of developing a serious knee problem. In a study reviewed by the National Institutes of Health, overweight people who lost an average of 11 pounds cut their risk of osteoarthritis in half.
  2. Are you exercising? Regular exercise is essential to maintaining knee strength. Without it, your muscles weaken, leaving your joints without ample support and leaving your muscles, bones, tendons, ligaments, and joints vulnerable to misalignment.

    Your best bet is to choose activities with a low risk of knee injury. A knee injury can double the risk of developing osteoarthritis. Daily moderate exercise is much better for your joints than occasional strenuous exercise. Focus on low-impact activities that build stamina, strength, and flexibility, such as yoga, walking, biking, swimming, and weight lifting. These types of exercise can help enhance circulation, improve your range of motion, and build the muscles that surround the knee joints. One study revealed that a relatively small increase in quadriceps strength (20%–25%) can lead to a 20%–30% decrease in the chance of developing knee osteoarthritis. Aim for a minimum of 30 minutes of exercise on most days of the week.

    Knee-Friendly Exercises

    Water workouts provide low-impact resistance and add a strength-training aspect to aerobic exercises such as walking or jogging. T'ai chi can help increase your range of motion, lengthen your muscles, and make your ligaments and tendons more resilient. Isometric exercises and yoga strengthen core body muscles as well as leg muscles that support the knee.
  3. Are you overusing some muscles and joints? Staying active is one of the best things you can do to protect your knees, but you should avoid repetitive strain on muscles and joints. For example, repeatedly engaging in the same activity - whether for work, recreation, or exercise — may loosen tendons or damage cartilage and eventually lead to injuries and possibly even arthritis.

    Determining if you are overusing a joint requires listening to your body. When you feel pain or discomfort during or after exercise, household chores, or other activities, don't ignore it. Take a break and consider ceasing the activity altogether until you can perform it without pain. In the meantime, stay active by focusing on other activities that do not stress the injured joint. If the pain does not go away in 2 weeks, see your healthcare provider.

    To help avoid overuse injuries, spend 5–10 minutes warming up before you exercise and another 5–10 minutes cooling down afterward.

Healthy people who take Nonsteroidal anti-inflammatory drugs (NSAIDs) to relieve minor aches and pains may raise their risk of dying from heart-related problems, a Danish study found a year ago.

The American Heart Association and the U.S. Food and Drug Administration already warn people with heart disease to be cautious about taking NSAIDs, which include ibuprofen (brand named Advil, Motrin) and diclofenac.

"Very few studies have been designed to answer the important question: Do NSAIDs also increase the cardiovascular risk among healthy people who use these drugs for minor complaints?" said lead author Dr. Emil L. Fosbol, a cardiologist at Gentofte University Hospital in Hellerup. "This study is the first to confirm that the cardiovascular risk is indeed increased when healthy individuals use some of the drugs."

The risks for different NSAIDs — found in an analysis of national medical records of more than one million Danes from 1997 to 2005 — varied widely. Participants, whose average age was 39, who used ibuprofen had a 29 percent greater risk of fatal or nonfatal stroke, compared to those who took no NSAID.

Use of diclofenac (Voltaren and Cataflam) was associated with a 91 percent higher risk of death from all cardiovascular diseases, while rofecoxib (Vioxx) use was associated with a 66 percent increased risk. But the study found no increased risk of cardiovascular problems — indeed, a slightly lower risk of death — associated with naproxen, sold over the counter with brand names including Aleve.

For people taking the largest doses, diclofenac was associated with a doubled risk of heart attack, and rofecoxib (Vioxx) was associated with a threefold increased risk of heart attack. Vioxx was taken off the U.S. market in 2004 because of a study finding high rates of heart attack and stroke.

The percentage increases in the study were large, but the absolute overall risk in otherwise healthy people was small, Farkouh said. Nevertheless,"before you take any medication, you should consult with a physician, particularly these medications," he said.

That warning applies especially to people who exercise regularly and are thus more likely to take an NSAID for muscle and joint pain, Farkouh said. Regular use of an NSAID increases the risk not only of cardiovascular problems but also of bleeding, a known side effect of the medications, he said.

I now always try to avoid taking NSAIDs for any kind of pain, preferring natural solutions, however at one time when adventure racing I used to consume Ibuprofen continuously every 4 hours during multi day races, perhaps I would not have done so if I knew of the increased risks, perhaps that can explain why there has been a few sudden deaths in the sport.
