Typically when we think of inflammation, we think of a body part becoming swollen from injury, like the swelling of a sprained ankle. But inflammation can also be caused by allergies to food, medications or environmental pollutants and can manifest in many different parts of the body. What is Palindromic rheumatism?

Palindromic rheumatism is just one of many. Palindromic rheumatism is a very rare type of inflammatory arthritis. Symptoms are often mistaken for and misdiagnosed as rheumatoid arthritis. Also, these two conditions can also overlap, existing in the same patient. That being said, however, there are still some distinguishing features between palindromic rheumatism and rheumatoid arthritis.

Below are some salient points about Palindromic Rheumatism,

  • Palindromic rheumatism is an inflammatory type of arthritis – since not a whole lot is known about it, it is often referred to as a syndrome instead of a disease like rheumatoid arthritis. A syndrome is a 'collection of symptoms.' Fibromyalgia is often also referred to as a syndrome.
  • Palindromic rheumatism is characterized by attacks of inflammation on joints, tissue, and muscle. It is not reserved for the joints only. These attacks come and go, and usually are "quicker" than typical RA or Lupus flares — albeit, very painful in nature. These attacks of palindromic rheumatism come on VERY SUDDENLY and spontaneously, and can last a few hours to a few days, usually affecting 1-3 areas. These attacks are usually intense, but are unpredictable in frequency and duration. Remission, like onset, is very sudden and spontaneous in nature.
  • It usually comes and goes from one joint to another. Example: In one attack, it could start in a thumb, move to the arm the next day and then hours later the arm will be better and it could move to another joint or tissue.
  • Palindromic rheumatism does not usually lead to permanent joint damage, like rheumatoid arthritis does. This is one of the main differences between the two diseases. However, as stated, palindromic rheumatism can occur in patients who do have rheumatoid arthritis also.
  • Subcutaneous nodules can develop in palindromic rheumatism patients. These nodules are unlike the nodules seen in RA patients, however.
  • X-rays are usually normal with palindromic rheumatism patients.
  • Sed-rate and rheumatoid factor tests may or may not be positive in palindromic rheumatism patients.
  • If a patient starts out with palindromic rheumatism, they have a better chance than the average person of developing RA. Sometimes palindromic rheumatism turns into rheumatoid arthritis.
  • About 30-40% of palindromic rheumatism patients' condition worsens over time. The attacks become more frequent and may last longer.
  • Only between 100,000 & 200,000 people in the United States have palindromic rheumatism, making it much less common than rheumatoid arthritis. Men and women are equally affected by palindromic rheumatism, another unique difference from rheumatoid arthritis which is more prevalent amongst females.
  • Palindromic rheumatism typically affects people from 20 years old to 70 years old and is not documented in children.
Treatment for Palindromic rheumatism usually consists of Nonsteroidal anti-inflammatory drugs, oral steroid use, steroid injections, and Disease-modifying antirheumatic drugs(which include adventure an anti-Malaria drugs). What is Chronic Inflammation?

This is inflammation that is not triggered in response to injury or infection. The body's normal system of protection and repair actually turns on itself and results in tissue damage and it is very important.

Almost half of all heart attacks occur in people with normal cholesterol. A marker for inflammation is a chemical called C-reactive protein. In 2002 the New England Journal of Medicine published a study in women showing a link between increased C-reactive protein level and an increase in heart problems like stoke and heart attacks. The researchers state that C-reactive protein was a stronger predictor of cardiovascular events than LDL cholesterol.

Research into Chronic Inflammation

In January 2005 there were 2 studies in the New England Journal of Medicine that show the potential benefits of lowering C-reactive protein levels. The first study took people who had suffered a heart attack and gave them drug therapy to lower cholesterol and C-reactive protein. They found those with lower cholesterol had a lower cardiac risk, but they also found those who had lower C-reactive protein levels also had lower cardiac risks. This benefit was independent of the benefit of lowering cholesterol.

In the second study, people who had atherosclerosis (partially blocked diseased arteries) were given therapy to lower cholesterol and C-reactive protein. There was less progression of artery disease in the group who had the lowest C-reactive protein. This benefit was independent of lowering cholesterol level. In the November 2009 edition of New England Journal of Medicine there was a medical study in which researchers treated apparently healthy people who had high C-reactive protein but normal cholesterol. One group was given medication to lower the C-reactive protein and the other was given a placebo. They were then followed to see if they would develop heart problems. The treated group suffered almost half the heart attacks and strokes as those who were not treated. The results were so significant, the researchers stopped the study 3 years early to offer treatment to all the study participants.

In the above studies C-reactive protein was reduced by cholesterol lowering drugs known as statins. These are well known to lower both cholesterol and more recently C-reactive protein. There is also evidence that diet, exercise, weight loss and quitting smoking can lower C-reactive protein and inflammation. Trans fats in the diet have been shown to increase C-reactive protein and inflammation.

Conclusion

While it may still be too early to test everyone for C-reactive protein and treat a high level, it certainly is something you should discuss with your doctor. If you are at a low risk for cardiovascular disease you may not need to worry about C-reactive protein levels. If you are at moderate risk, knowing this level may help to direct your treatment. A high C-reactive protein may indicate more aggressive therapy is needed Finally, if you are at high cardiovascular risk, you need aggressive cholesterol reduction therapy which in turn will help lower C-reactive protein. Whatever your situation, eat a healthy low-fat diet with lots of fruits and vegetables and follow an exercise program. A healthy lifestyle will help your medication achieve the best possible results. How Much Extra Risk?

Nearly a decade ago, at the American College of Rheumatology Annual Scientific Meeting in October/November 2000, researchers reported that the risk of developing rheumatoid arthritis is nearly double for current smokers compared to non-smokers. The risk of developing rheumatoid arthritis is lower for former smokers compared to current smokers, but higher than for people who never smoked. In one study, women who were smokers but had stopped at least 10 years prior to the start of the study did not have an increased risk. however other studies indicate the risk remains high even after 20 years.

Are Men or Women More Affected?

From a meta-analysis of 16 studies, it was concluded that the strongest association between smoking and rheumatoid arthritis occurred in men who were positive for rheumatoid factor. When parameters were limited to rheumatoid factor positive patients, men had double the odds ratio of women.

Should You Stop Smoking?

Study results, presented at American College of Rheumatology Annual Scientific Meeting in October 2008, showed that measures of active disease — including swollen and tender joint counts and C-reactive protein — were lower in patients who had stopped smoking. This suggests that, even after you have rheumatoid arthritis, if you stop smoking you can lessen rheumatoid arthritis disease activity.

"All of the heat absorbed during the day should dissipate and steadily decrease throughout the night before reaching its lowest point just before the dawn and coming back up," he says. "Sleep is a release of energy." Scientists have found, though, that people suffering from sleep disorders don't get cool enough at night because they're inflamed. In some cases, their body temperature literally qualifies as a fever state (over 37ÂșC), says Naiman. Naiman has a few tips for tho se who suffer from the occasional bout of insomnia and are looking for ways to get a better night's sleep:

  • Cool the room: Lower your thermostat at night to around 20 C.
  • Turn off the TV: A little TV humour is okay before bed, but avoid being "dramatized" by one-hour police shows or murder mysteries. Save those for the day, says Naiman. At night you want to create a release of energy, not a buildup.
  • Unplug your brain: Don't take or make any phone calls or fire off any emails one hour before sleep.
  • Prep for rest: Use your pre-sleep hour to read a book or practice simple relaxation techniques like meditation and gentle yoga.
  • Set your alarm: Move your clock to somewhere you can hear it, but not see it. And if your clock has a digital display, ditch it. "Even the small amount of light from clock radios can trickle through closed eyelids, suppressing melatonin production and disrupting sleep," says Naiman.

2. Relationship stress

Have you been burned by love? New evidence shows that stress from personal relationships directly impacts our body's inflammatory response. In fact, love gone wrong can sometimes be as bad for the body as smoking, obesity and hypertension. And, once again, women are more at risk than men because we produce more of the stress hormone cortisol than they do, and it does more damage in our bodies. Recent studies showed that women with lower marital satisfaction experience a more rapid progression of inflammation-related disorders like carotid atherosclerosis (a vascular disease affecting the arteries to the brain), arthritis and metabolic syndrome, which can lead to heart disease and diabetes. Part of this is because trusting, supportive relationships can inspire health-promoting behaviours, including better eating habits and romantic walks together, while unsupportive relationships can result in cancelled trips to the gym, more sleep disturbances, Ice cream binges and higher rates of smoking and drinking. But it's even more subtle than that: Often, our expectations about the availability and responsiveness of our partners can influence how the body reacts to stress, especially when expectations fall short. This can lead to depression, which studies show makes us more vulnerable to infection, fuels inflammatory production and may slow down the healing process. One 2009 study from Ohio State University also found that how we love directly affects our immune systems. People who avoid conflict or withhold love (an act dubbed "attachment avoidance") have higher levels of inflammation than those who are less evasive. In other words, experiencing your feelings, working through your problems and being open-hearted is better for your body and psyche than bottling up your feelings or pushing someone away. What's more, when women avoid intimacy their cortisol levels spike—along with their risk of chronic inflammation.

3. Forgetting to clean your teeth

"The old Brush your teeth should really be Clean your mouth," says Howard Tenenbaum, head of research in the Department of Dentistry at Mount Sinai Hospital in Toronto. Ever since a bidirectional link was discovered between gum inflammation and Type 2 diabetes, and, possibly heart disease, periodontists have been sharing patients with endocrinologists and cardiologists. "We now know that treating periodontal disease—swollen and inflamed gums that bleed easily and may have a slightly bluish tinge—can improve the clinical course of diabetes," says Tenenbaum. Although rumour has it that cleaning your mouth properly can also ward off heart disease, here the evidence is more tenuous. "There's a strong possibility of a link," says Tenenbaum, ™but we don't know for sure." So why is a clean mouth so vital to our overall health? "It's the only area of the body where a structure—the teeth— perforates the skin," says Tenenbaum. " ;It's a weak spot and a breeding ground for bacteria." When gums are diseased they spill a whole array of cytokines into the bloodstream. Flossing and using a soft brush in circular motions along the gum line is key to eliminating bacteria and warding off inflammation, but so is brushing your tongue and sweeping the bristles over the insides of your cheeks. Tenenbaum recommends toothpastes that contain triclosan , an antibacterial ingredient, as well as antibacterial rinses. Tenenbaum's lab is now conducting a clinical trial with a rinse containing resveratrol, a compound in red wine that has been shown to block inflammation.

This can cause your eyes to feel gritty and dry. If not treated, dryness can lead to infection and scarring of the conjunctiva and cornea.

    Many people with rheumatoid arthritis have a collection of fluid between the pericardium and the heart itself (called a pericardial effusion), but not all will have clinical symptoms from it. Episodes of pericarditis usually develop during flares, or periods of heightened disease activity.

    Persistent pericarditis can lead to thickening and tightening of the membrane, which can interfere with the heart's ability to work properly. Lesions similar to rheumatoid nodules can also develop on the heart and affect heart function. Inflammation of the heart muscle itself (myocarditis) is a Rheumatoid Arthritis complication.

    Drugs used for Rheumatoid Arthritis treatment may also weaken the heart and other muscles.

Although different problems may require different doctors and/or different treatments, often a change in arthritis treatment — either to better control the disease or to eliminate the side effects of a certain treatment — can be an important step in managing or resolving all of these problems.

Researchers at Columbia University and University of Miami discovered that although more Americans are working past age 65 by choice, a growing segment of the population must continue to work well into their sixties out of financial necessity. Researchers looked at aging, social class and labour force participation rates to illustrate the challenges that lower income workers face in the global marketplace. The study used the burden of arthritis to examine these connections because 49 million U.S. adults have arthritis, and 21 million suffer activity limitations as a result. The condition is also relatively disabling and painful but not fatal. The researchers found that blue collar workers are much more likely to work past 65 than white collar workers and are much more likely to suffer from conditions like arthritis, reducing their quality of life and work productivity.

The study findings are reported online in the American Journal of Public Health, and will be published in September,

The investigators calculated estimates and compared age-and occupational specific data for workers with and without arthritis, merging data from the U.S. National Health Interview Survey (NHIS), Medical Expenditure Panel Survey (MEPS) and National Death Index. They studied 17,967 individuals for the analysis out of 38,473 MEPS participants.

"Arthritis serves as a powerful lens for looking at these convergent phenomena," said Alberto J. Caban-Martinez, DO, PhD, MPH, Department of Epidemiology and Public Health at the University of Miami Miller School of Medicine and first author. "We found that blue-collar workers with arthritis are in much worse health than are all other workers, suggesting that they are struggling to stay in the workforce despite their health condition."

At all ages, blue-collar workers in the workforce are in worse health than white-collar workers. By age 65, 19% of white-collar workers with arthritis remain in the workforce compared with 22% of blue-collar workers. But employed blue-collar workers have more severe disease than employed white-collar workers, and look forward to fewer years of healthy life -- approximately 11 for blue-collar workers and 14 for white-collar workers.

The investigators reported that lower-income workers of older age in the service and farming sectors–– two job types that are unlikely to come with pension plans––are more likely to have arthritis than not, with 58% of service workers and 67% of farm workers continuing to work despite struggling with the painful health condition. Sixteen percent of all blue collar workers are over 65 and 47% report they have arthritis. By contrast, 14% of white collar workers work beyond the age of 65, and 51% of these workers reporting arthritis. Overall, approximately 15% of all workers remain in the workforce at or past retirement age, and 44% have arthritis.

"The increasing age of the U.S. workforce presents new challenges for government, employers and working families," observes Peter Muennig, MD, MPH, associate professor of Health Policy and Management and senior author. "It is estimated that by the year 2030 approximately 67 million adults aged 18 years and older will have arthritis. Because the 'graying' workforce will be disproportionately represented by people from middle and lower occupational classes that also suffer from a higher prevalence of arthritis and a shorter life expectancy than wealthier Americans", Dr. Muennig points out that additional enhancements to federal programs such as better disability, health and unemployment insurance will be needed to maintain a higher quality of life for all workers, particularly for those with chronic conditions such as arthritis. "As the population ages in the face of expanding budget deficits, we face politically difficult choices if the U.S. is to p revent significant declines in its standard of living".

Pharmaceutical companies get richer and sadly we grow more dependent and addicted to higher doses on the so-called "pain-killers". Who said health is wealth the money is in being sick for these companies. What I call a numbing effect so it rarely addresses the root cause of the pain. You don't want to feel pain but it's an important part in the healing process. Your body is trying to communicate with you: If you feel pain, it means that your central nervous system thinks the body is under threat, and that something has to be done about it. In this sense, pain is a survival mechanism of fundamental importance. You may not know this but you don't feel pain until the brain interprets the information and decides that pain would be a good way to encourage you to take action that will help protect and heal the damage. So numbing these signals does little to help you take action, always follow your doctor's advice but ask questions if there are alternatives to taking "pain-killers"?

In General there are too many side effects for me to list here but two extreme examples that I was even shocked by would be.

  • Acetaminophen has few side effects. If taken in very high doses, for extended periods of time or with excessive use of alcohol, this medication can lead to liver damage.
  • Opioids are used in medicine as strong analgesics (pain-killers), for relief of severe or chronic pain. Interestingly, there is no upper limit for the dosage of opioids used to achieve pain relief, but the dose must be increased gradually to allow for the development of tolerance to adverse effects (for example, respiratory depression). According to eMedicine, "Some people with intense pain get such high doses that the same dose would be fatal if taken by someone who was not suffering from pain."
Alternatives to avoid pain medication, a therapeutic massage (neuromuscular therapy), feldenkrais, chiropractic, yoga, pilates, tai chi, acupuncture, etc. These work by creating noise to block the pain signals to your brain.

A recent study on low back pain concludes that general massage therapy is more effective than standard medical treatment when dealing with back pain. This means that a weekly massage can do more to ease chronic back pain than painkillers, anti-inflammatory drugs (think Advil) and physical therapy, good news for everybody out there.

Many people turn to over-the-counter (OTC) pain relievers when seeking relief from pain and inflammation, these are medicines that you can buy without a prescription from your doctor. Two main types of OTC pain relievers are available. One type is acetaminophen (one brand name: Tylenol). The second type is non steroidal anti-inflammatory drugs (also called NSAIDs). NSAIDs include the following:

Some products contain both acetaminophen and aspirin (brand names: Excedrin Extra Strength, Excedrin Migraine, Vanquish).

Acetaminophen seems to relieve pain and reduce fever by working on the parts of the brain that receive pain messages and control the body's temperature.

NSAIDs relieve pain and fever by reducing the level of hormone-like substances (called prostaglandins) that your body makes. These substances cause the feeling of pain by irritating your nerve endings. They also are part of the system that helps your body control its temperature.

Acetaminophen and NSAIDs help to reduce fever and relieve pain caused by headaches, muscle aches and stiffness. NSAIDs can also reduce inflammation (swelling). Acetaminophen does not reduce inflammation.

OTC pain relievers can be helpful in treating many types of pain, such as pain from arthritis, earaches, back pain, and pain after surgery. Children who may have the flu or chickenpox should not take aspirin because they are at higher risk to develop a condition called Reye's Syndrome.

Acetaminophen can be a good choice for relieving headaches and other common aches and pains. It can be used safely on a long-term basis by most people for arthritis and other chronic painful conditions.

Ibuprofen is helpful for relieving menstrual cramps and pain from inflammation (such as muscle sprains). If ibuprofen doesn't work for you, Naproxen may be an option.

Side effects of OTC pain relievers are rare for healthy adults who only use pain relievers once in a while. If you have health problems or use OTC pain relievers often, talk with your family doctor.

Side effects with acetaminophen are rare. However, liver damage can occur if you drink alcohol and take acetaminophen.

NSAIDs may cause upset stomach. They can also cause increased bruising or risk of bleeding in the stomach. When taken regularly, they may cause kidney damage. NSAIDs may also make high blood pressure worse.

Do not take acetaminophen if you:

  • Have severe kidney or liver disease
  • Have 3 or more drinks that contain alcohol every day

Prickly pears are members of the Cactacceae or cactus family, which includes thousands of species. The cacti are found in Europe, Mediterranean countries, Africa, southwestern United States, and northern Mexico. Plants in the genus Opuntia prefer a dry, hot climate and consist of perennial shrubs, trees, and creeping plants. Prickly pear can grow 5 to 8 m in height; its roots are shallow, but the plant can spread up to 40 m in diameter over the ground. The stems are branched, leaves are cylindrical in shape, and the plant is covered with barb-tipped bristles (known as glochids) that are unique to Opuntia cactus . Its flowers, petals, and sepals are numerous in quantity and colour. The oval, pear-shaped, purplish fruit is pulpy and sweet but may be covered with spines or bristles. The seeds within the pulp are disk-shaped and have numerous colours.

The prickly pear cactus is widely cultivated and used in juices, jellies, candies, teas, and alcoholic drinks. The fruits and flowers of the plant are used as natural food colorants. Cactus gum is used to stiffen cloth. Essential oils from the flowers are used to make perfumes, and the seeds are a source of oil. Prickly pear has also been used as a source of animal feed and dye.

There are many medicinal uses of the cactus. American Indians used prickly pear juice to treat burns. Often a cone of plant material would be burned on the skin to treat irritation or infection, a process known as moxabustion in Chinese medicine. The Lakota tribe used prickly pear in a tea to assist mothers during childbirth.

Prickly pear cactus has a long history of traditional Mexican folk medicine use, particularly as a treatment for diabetes. Prickly pear pads have been used as a poultice for rheumatism. The fruit has been used for treating diarrhoea, asthma, and gonorrhoea. The fleshy stems or cladodes have been used to treat high cholesterol, blood pressure, gastric acidity, ulcers, fatigue, dyspnea, glaucoma, liver conditions, and wounds. In South Korea the plant has been used to treat abdominal pain, bronchial asthma, burns, diabetes, and indigestion. In Sicily, a flower decoction of prickly pear has been used as a diuretic; the cladodes were valued for their anti-inflammatory activity in treating edema, arthrosis, and whooping cough, and for preventing wound infection.

When most people think of different types of back pain, usually there definition is something along the lines of, "ouch" back pain, I can't pick up the newspaper" back pain, or "I've fallen and I can't get up" back pain. Although descriptive and identifiable, it is important to have an idea of what type of pain you are experiencing so you can go through the proper channels to get help.

There are two main types of back pain, acute pain and chronic pain. Both have different severities to the types of pain you are experiencing. As is the case with all types of pain, both also have different treatments to get you back on your feet and pain free, but first you need to identify the problem.

The first common type of back pain is acute pain; this is pain that lasts less than 3 to 6 months, or pain that is directly related to tissue damage. There are a few different levels of acute pain, the lowest level might be similar to something like a paper cut. This pain will cause a fast, immediate, intense pain with an almost simultaneous withdrawal of the body part that is causing pain. More of an aching pain might be experienced a few seconds after the initial pain and withdrawal. The next level would come from something like dropping a weight on your foot. This pain is similar to that of the paper cut in that there is immediate pain, withdrawal and then slower aching pain. The strongest level would be something like labour pains or pain from kidney stones. The pain during childbirth is acute and the cause is certainly identifiable.

The longer pain goes on, the more susceptible it is to other influences and developing into the next form of back pain called chronic pain. Chronic pain can be defined by one rather annoying word, persistence. Pain signals keep firing in the nervous system for weeks, months, even years. Chronic pain is measured by duration, pain that persists for more than 3 months is considered chronic. There may have been an initial mishap like a sprained back or a serious infection. Another cause of chronic pain might be an ongoing problem like arthritis, cancer or even something smaller like an ear infection. In some cases, people can even suffer chronic pain in the absence of any past injury or evidence of body damage. This pain is often progressive and is difficult to determine.

I myself have Arthritis but no longer suffer chronic pain since I began taking just one ounce of Nopalea cactus juice on a daily basis, it is quite amazing and has been advertised on TV.

They are both low impact and easy on the joints. If you take an aquafit class, you will get both resistance training and cardio all in one. If you find walking difficult because you're unstable on your feet, try Nordic walking. With the aid of walking poles, Nordic walking is very popular among arthritis sufferers and older people who are not so steady on their feet.

Staying flexible is crucial for arthritic sufferers and engaging in some stretching or Yoga classes are both ideal ways to stay limber. Resistance training using resistance bands twice a week or more is a gentle way to keep the supporting muscles around your joints strong and supple.

Here are a few exercises that focus on flexibility and range of motion. These can be done with or without bands:

  • Shoulders – start with your arms at your sides and bring them all the way up toward the ceiling. Continue as far behind your body as comfortable, making a huge circle. Repeat 10 – 20 times.
  • Wrists – Make circles with your wrists, rotating your hands in both directions. Repeat 10 times in each direction.
  • Ankles – While sitting comfortably in a chair, draw big circles in the air with your big toe. Repeat 10 times both clockwise and counterclockwise.
  • Hips – Start by lying on your back on a firm, but comfortable surface. Your knees will be bent, and your feet flat on the floor, with your arms down by your sides. Bring one knee to your chest, slowly rotate the knee in an easy circle. Repeat 5 – 10 times with each knee.
